Since new when the engine is cold in the morning, it makes a strange light rattle noise under acceleration, it goes away once the engine gets to temp. Gently push the linkage rearward to take the pressure off and turn the idle screw counter clockwise (up) to increase idle speed, turn the idle screw clockwise (down) to decrease idle speed. Some tests require special tools and if you don't have access to the tools, I would suggest replacing the fuel control actuator, since it's the most common cause for this. Loosen the 10 mm Locknut on the low idle adjusting screw at the rear of the injector pump. It sounds like you have a faulty fuel control actuator and without monitoring the pressure with a scan tool or performing some fuel system tests, I can't say for sure this will fix your problem. The most common reasons a Ram 3500 has rough idle are a vacuum leak, an issue with the spark plugs, or a problem with the ignition coil. It sets when there is a deviation between the fuel pressure setpoint and actual pressure. P0148 is likely the code that is related to your problem. If you don't have a aftermarket programmer on your vehicle, it could be caused by a faulty map sensor. P0234 will set when the boost pressure is exceeded and is usually caused by aftermarket performance programmers. When the oil temp and IAT are cold, the EBPV will close to put a tiny bit of load on the engine. Another part of the CAP is control of the exhaust back pressure valve (EBPV). I don't think this code is related to your problem. When the oil temp and the intake air temp (IAT) are cold enough, the engine will automagically idle up to a faster RPM when in park or neutral for more than a minute or so. P2509 will set when the ECM loses power and is usually caused by weak batteries or something drained the batteries down.
